Job Overview

Social worker is responsible for demonstrating the mother standard of care by providing support services to patients and their families in the management of psychosocial implications and impact of illness.  The social worker acts as a resource assisting with communication, counseling, navigation of systems of care, providing resources, and providing discharge planning of services to the patient and family.  The social worker will assist the patient and family of all ages including children, adults, and geriatrics.  Considered an integral part of the clinical care team, participating in multidisciplinary meetings, communications, and collaboration as part of patient care with the physician and allied health professionals.  Conducts comprehensive discharge needs assessment, initiates and facilitates referrals to home health, hospice, other care facilities, and medical equipment and supplies.  Implements plan of care using psychotherapeutic intervention; provides crisis intervention including adjustment to hospitalization, disability, chronic illness, mental health illness, abuse, and financial counseling including assistance with disability applications.  Assist with ensuring safety of patients.  Upholds all regulatory agency requirements including The Joint Commission and State Department of Health. 

Job Qualifications

  • Masters of Social Work Required
  • 3 years social work experience required
  • Current BLS certification
  • Demonstrate ability to utilize critical thinking and apply sound judgment and assessment skills for decision making, and ensuring safety of patients
  • Excellent  written and verbal communication skills
  • Ability to interact with a wide variety of individuals and handle several situations simultaneously
  • Microsoft outlook, electronic health record experience required
  • Lifting/carrying objects up to 50lbs
